Wellsprings Frequently Asked Questions
|
Because there are many insurance carriers and types of coverage (e.g. HMO, PPO, POS), the answer to this question depends specifically upon your insurance policy. Please contact us with the following information, and we will provide you with a prompt response to this question:
- The name of the person seeking services
- The type of services being sought (individual, couples, group, family)
- The date of birth of the person seeking services
- The name of the insurance company
- The insured's name (if different than above)
- The subscriber's id
- The insurance company's phone number (for behavioral health services)
Use the insurance portion of our contact form to provide the information needed for us to check your insurance.
We urge you to check on your insurance benefits personally before contacting us. We are generally out of network with most plans. For purposes of checking specific benefit information, we would be considered "out-patient mental health" services.
For your convenience, we submit to insurance on your behalf, however the responsibility of payment and insurance reimbursement is yours.
